What is Momentum?
Momentum is the practical, relational support offered by The Horizons Project between appointments with the services already involved in your life.
It is not counselling or therapy, and it does not replace professional support. Instead, it can provide encouragement, continuity and practical prompts that help you hold onto progress when circumstances feel difficult or overwhelming.
Could Momentum be right for me?
Momentum may be helpful if you are:
- finding it difficult to keep moving forward;
- feeling overwhelmed by appointments or life circumstances;
- struggling to remain engaged with existing services;
- looking for practical encouragement between appointments;

Wherever appropriate, Horizons works alongside existing services rather than attempting to replace them.
Common questions
Do I need a referral?
The final referral and access arrangements will be confirmed as the pilot develops. This page can be updated once those arrangements have been formally approved.
Is Momentum confidential?
Your privacy will be treated respectfully. The limits of confidentiality, consent and any information-sharing arrangements will be explained clearly before support begins.
What happens after I make contact?
The first step is an informal conversation. It gives you an opportunity to ask questions and helps both you and Horizons consider whether Momentum is an appropriate next step.
What if Momentum is not suitable for me?
Horizons will be honest about its remit and limitations. Where possible, you may be encouraged to speak with a more appropriate service or professional.
